After it self-disclosed conduct to OIG, Personalized Therapy, LLC (Personalized Therapy), Maryland, agreed to pay $334,519.37 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that Personalized Therapy improperly billed Tricare for services provided by one Occupational Therapist Assistant when those services were not reimbursable.
