After it self-disclosed conduct to OIG, Outreach Health Services of Oklahoma, LLC (Outreach), Oklahoma, agreed to pay $15,000 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that Outreach submitted claims to Oklahoma Medicaid for home health services provided at two locations while those locations operated without required state licenses.
