Michael Lesem, MD and Michael D. Lesem, MD PA Agreed to Pay $546,000 for Allegedly Violating the Civil Monetary Penalties Law by Submitting Claims for Bundled Services that Included Drugs that Were Not Purchased

On November 20, 2025, Michael Lesem, M.D. (Dr. Lesem) and Michael D. Lesem, MD PA (Lesem MD PA), Houston, Texas, entered into a $546,954.46 settlement agreement with OIG. The settlement agreement resolves allegations that Dr. Lesem and Lesem MD PA submitted claims to Medicare Part B and Medicare Part C for the provision and administration of Spravato under HCPCS Codes G2082 and G2083 when they did not purchase the Spravato. Instead, specialty pharmacies delivered the Spravato to Lesem MD PA and submitted claims for the Spravato to Medicare Part D.
