Mantey Heights Rehabilitation & Care Center Agreed to Pay $540,000 for Allegedly Violating the Civil Monetary Penalties Law by Submitting Claims that Were Falsely Signed by an Individual who Lacked Proper Credentials

After they self-disclosed conduct to OIG, SNH CO Tenant LLC d/b/a Mantey Heights Rehabilitation & Care Center (Mantey Heights); Diversified Healthcare Trust (formerly known as Senior Housing Properties Trust); Five Star Quality Care– Colorado LLC; FVE Managers, Inc.; and Five Star Senior Living Inc. (now known as AlerisLife), Colorado, agreed to pay $540,631.40 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that Mantey Heights’ MDS coordinator, a licensed practical nurse, signed Section Z0500 electronic health records in Mantey Heights’ system using the login credentials for the then director of nursing. Section Z0500 is required to be signed by a registered nurse by the Medicare Part A program, the Medicaid program, and the Veteran’s Health Administration.
