Lake Minnetonka Shores Agreed to Pay $113,000 for Allegedly Violating the Civil Monetary Penalties Law by Submitting Claims for Services Provided by an Unlicensed Individual
After it self-disclosed conduct to OIG, PHS Lake Minnetonka LLC d/b/a Lake Minnetonka Shores (LMS), Spring Park, Minnesota, agreed to pay $113,091.91 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that LMS submitted claims to Federal health care programs for services provided by an unlicensed registered nurse. Single damages were calculated as the full salary and benefits paid to the nurse during the period they worked without a valid license.
