InSite Diagnostic Health Care Agreed to Pay $4.9 Million for Allegedly Violating the Civil Monetary Penalties Law by Submitting Claims that Incorrectly Identified the Rendering Provider
After it self-disclosed conduct to OIG, InSite Digestive Health Care (InSite), California, agreed to pay $4,934,727 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that InSite submitted claims to Medicare, Medicaid, and Tricare that misused the Q5 modifier and listed the incorrect name of the rendering provider on the claims.
