Initium Health Agreed to Pay $580,000 for Allegedly Violating the Civil Monetary Penalties Law by Inappropriately Drawing Down Funds from an HHS Grant

On December 4, 2025, Initium Health (Initium), Denver, Colorado, entered into a $580,000 settlement agreement with OIG.  The settlement agreement resolves allegations that Initium used the HHS Payment Management System to draw down funds from a Health Resources and Services Administration award to conduct COVID-19 vaccine outreach that: (1) were used to pay expenses that were unrelated to the purposes of the award; (2) lacked adequate documentation; and (3) were used in a manner that violated HHS’s and Initium’s conflict of interest rules.
