After it self-disclosed conduct to OIG, Hopebridge, LLC (Hopebridge), Indianapolis, Indiana, agreed to pay $25,336.48 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that Hopebridge submitted claims for plans of care that were not individualized to the patient.
