After it self-disclosed conduct to OIG, Elevation Hospice of Colorado, LLC (EHC), Colorado, agreed to pay $150,169.80 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that EHC submitted claims for services provided by two unlicensed individuals.
