After it self-disclosed conduct to OIG, ClearChoiceMD, PLLC (ClearChoiceMD), Vermont, agreed to pay $37,447.11 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that ClearChoiceMD submitted claims for certain evaluation and management services by a Nurse Practitioner that were not rendered.
