City of Fort Dodge Agreed to Pay $32,000 for Allegedly Violating the Civil Monetary Penalties Law by Submitting Claims for Services Provided by an Unlicensed Individual

On July 18, 2025, City of Fort Dodge (CFD), Fort Dodge, Iowa, agreed to pay $32,525.91 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that CFD submitted claims to Federal health care programs for services provided by an unlicensed paramedic.
