Beaver Dam Fire Department Agreed to Pay $29,000 for Allegedly Violating the Civil Monetary Penalties Law by Submitting Claims for Services Provided by an Unlicensed Paramedic

After it self-disclosed conduct to OIG, the City of Beaver Dam, et al. d/b/a Beaver Dam Fire Department (Beaver Dam), Wisconsin, agreed to pay $29,935.10 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that Beaver Dam submitted claims for ambulance transportation services provided by an improperly licensed paramedic.
