All Star Physical Therapy Specialists, Confluent Health, and Confluent Health California Agreed to Pay $636,000 for Allegedly Violating the Civil Monetary Penalties Law by Submitting Claims for Services by Non-enrolled or Uncredentialed Providers

After they self-disclosed conduct to OIG, All Star Physical Therapy Specialists, LLC (a/k/a All Star MSO, LLC), Confluent Health, LLC and Confluent Health California, LLC (collectively, “All Star”), with multiple locations in California, agreed to pay $636,118.16 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that All Star submitted claims to Federal health care programs that improperly claimed to have been furnished by a credentialed physical therapist when, in fact, another physical therapist actually rendered the services and the rendering physical therapist was not enrolled or credentialed with the applicable Federal health care program at the time the services were provided.
