All Star Physical Therapy Agreed to Pay $1.2 Million for Allegedly Violating the Civil Monetary Penalties Law by Submitting Claims for Services by Non-enrolled or Uncredentialed Providers

After it self-disclosed conduct to OIG, All Star Physical Therapy, Inc. (All Star PT), with multiple locations in California, agreed to pay $1,239,599.66 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that All Star PT submitted claims to Federal health care programs that improperly claimed to have been furnished by a credentialed physical therapist when, in fact, another physical therapist actually rendered the services and the rendering physical therapist was not enrolled or credentialed with the applicable Federal health care program at the time the services were provided.
