After it self-disclosed conduct to OIG, Five Star Quality Care-CA, LLC d/b/a Van Nuys Healthcare Center (VNHC), California, agreed to pay $1,138,807.29 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that VNHC's Director of Nursing and Minimum Data Set (MDS) Coordinator reported false Assessment Reference Dates on MDS reports required under Medicare rules.
