Supreme Menthal Health Services and Nicolas Calzadilla Mendez Were Excluded for 20 Years

Effective August 21, 2026, OIG excluded Supreme Menthal Health Services. Inc. (Supreme) and Nicolas Calzadilla Mendez (Mendez), Miami Lakes and Miami, Florida respectively, from participation in all Federal health care programs for a period of twenty years under 42 U.S.C. 1320a-7(b)(7). During all times relevant to the exclusion, Mendez owned, operated, or controlled Supreme. OIG imposed the exclusion based on Mendez and Supreme submitting and causing the submission of claims for B-12 injections, residence visits, and supervision and provision of the drug Spravato when the physician listed on the claims did not provide the services as claimed and was not associated with Supreme on the dates the services were purportedly rendered.
