After they self-disclosed conduct to OIG, Providence Health & Services-Oregon, Providence Health Assurance (PHA) and Providence Health Plan (PHP), Oregon, agreed to pay $201,477 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that PHA and PHP employed an individual that they knew or should have known was excluded from participation in Federal health care programs
