Masiel Chaviano Huerta Was Excluded for 20 Years

Effective August 17, 2026, OIG excluded Masiel Chaviano Huerta (Huerta), Hialeah, Florida, from participation in all Federal health care programs for a period of twenty years under 42 U.S.C. 1320a-7(b)(7). During all times relevant to the exclusion, Huerta owned, operated, or controlled Supreme Menthal Health Services Inc. (Supreme). OIG imposed the exclusion based on Huerta and Supreme submitting and causing the submission of claims for B-12 injections, residence visits, and supervision and provision of the drug Spravato when the physician listed on the claims did not provide the services as claimed and was not associated with Supreme on the dates the services were purportedly rendered.
