Liberty County Hospital District No 1 and PF Carthage SNF Ops Agreed to Pay $525,000 for Allegedly Violating the Civil Monetary Penalties Law by Employing an Excluded Individual

After they self-disclosed conduct to OIG, Liberty County Hospital District No. 1 (Liberty), and PF Carthage SNF Ops, LLC (PF Carthage), Carthage, Texas, entered into a $525,882.88 settlement agreement with OIG. The settlement agreement resolves allegations that Liberty and PF Carthage employed an individual that they knew or should have known was excluded from participation in Federal health care programs.
