Evergreen Home Healthcare Agreed to Pay $36,000 for Allegedly Violating the Civil Monetary Penalties Law by Submitting Claims for Home Health Services that Were Not Rendered

After it self-disclosed conduct to OIG, Evergreen Home Healthcare, LLC (Evergreen), Colorado, agreed to pay $36,105.44 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that Evergreen submitted claims for reimbursement to the Colorado Medicaid program for home health services furnished by one of its employed certified nursing assistants, to her minor child, that were not rendered.
