City of Tempe Agreed to Pay $1 Million for Allegedly Violating the Civil Monetary Penalties Law by Submitting Claims for Services Provided by Unlicensed Individuals and Services Containing Invalid Signatures

After it self-disclosed conduct to OIG, City of Tempe (Tempe), Arizona, entered into a $1,050,629.81 settlement agreement with OIG. The settlement agreement resolves allegations that Tempe submitted claims for services provided by unlicensed emergency technicians and paramedics. OIG further contended that Tempe submitted claims to Federal health care programs for services provided by a former paramedic where the paramedic inappropriately signed authorization forms for treatment, privacy, transport, and billing on behalf of the patients or their legal representatives.
