After it self-disclosed conduct to OIG, Bridge Crest Post Acute (Bridge Crest), Vancouver, Washington, entered into a $43,519.13 settlement agreement with OIG. The settlement agreement resolves allegations that Bridge Crest submitted claims to Federal health care programs for services provided by a Licensed Practical Nurse with an expired license.
