After it self-disclosed conduct to OIG, Bret Gene Ball, LLC (Ball), Oregon, agreed to pay $438,493.44 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that Ball submitted claims to Federal health care programs for services provided by a physician assistant under Dr. Bret Gene Ball’s National Provider Identification Number as if the services were rendered by Dr. Bret Gene Ball.
