After it self-disclosed conduct to OIG, Amherst SNF Operations d/b/a Fairmont Crossing Health and Rehabilitation (Fairmont), Virginia, agreed to pay $40,394.13 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that Fairmont submitted claims to Medicare and Medicaid for services provided by an unlicensed certified nursing assistant. Single damages were calculated as the full salary and benefits paid to the nurses during the period they worked without a valid license.
