After it self-disclosed conduct to OIG, Jeffrey K. Chaulk, M.D., P.C. d/b/a Alpine Eye Care (AEC), Michigan, agreed to pay $23,601.11 for allegedly violating the Civil Monetary Penalties Law. OIG alleged that AEC submitted claims to Medicare for services that were medically unnecessary or improperly coded.
