Payments for Epogen Administered at Fresenius Medical Care-First State, New Castle, Delaware

Executive Summary

Between January 1, 2004, and June 30, 2006, Fresenius Medical Care-First State did not meet Medicare payment requirements for some dates of service for 25 of the 100 claims reviewed and received an overpayment of at least $7,000. Also, First State policies and procedures were not always followed.
