Between January 1, 2004, and June 30, 2006, Bon Secours Renal Dialysis Center did not meet Medicare payment requirements for some dates of service for 5 of the 100 claims reviewed and received an overpayment of $89. Also, Bon Secours policies and procedures were not always followed.
