Executive Summary
Between January 1, 2004, and June 30, 2006, Fresenius Medical Care-Beckley did not meet Medicare payment requirements for some dates of service for 28 of the 100 claims reviewed and received an overpayment of $26,000. Also, Beckley policies and procedures were not always followed.
